After contacting Shelley to book your appointment, the next step is to print and complete the intake package below.

Please complete the paperwork to the best of your ability so that we can use our first meeting as efficiently as possible. When we meet, I will review the information you have provided and you will have the opportunity to clarify or explain any details. There are no wrong answers but the more honest you are the more appropriate and extensive my recommendations will be.
